(AP) ? State officials investigating a pharmacy linked to a deadly outbreak of meningitis said Tuesday they found shoddy sterilization practices and unclean conditions there, including debris-covered floor mats and standing water from a leaking boiler.

State officials also said the New England Compounding Center shipped steroids from the possibly contaminated batches suspected in the outbreak before it received its own test results confirming the drugs were sterile.

Gov. Deval Patrick said he's ordered state pharmacy regulators to conduct surprise inspections ? the first of which happened Tuesday ? at companies similar to the NECC and take other steps to tighten oversight. The state also has moved to revoke the company's operating license and the licenses of its top three pharmacists.

"Those whose laboratory practices caused this outbreak should never practice pharmacy or manufacture in Massachusetts again," Patrick said.

The outbreak of fungal meningitis, an inflammation of the lining of the brain and spinal cord, has sickened 308 people, including 23 who have died, in 17 states. The outbreak has been linked to a steroid made by the NECC and taken mainly for back pain. Compounding pharmacies like NECC custom mix solutions in doses or forms generally not commercially available.

The federal government is conducting a criminal investigation.

The state said Tuesday that its preliminary investigation, which began last month after the company was first suspected in the growing outbreak, found batches of drugs ready for general distribution but not labeled for specific patients.

Its state license permits the company to fill out only specific prescriptions for specific patients, and distributing drugs in batches like a manufacturer would violate that, said Dr. Madeleine Biondolillo, director of the state Department of Public Health's Bureau of Healthcare Safety.

But company attorney Paul Cirel said it's "hard to imagine" state regulators weren't previously aware of the scale of its operations because they've worked so closely together. The state Board of Registration in Pharmacy has always had complete access to the facility, and board members were there as recently as last summer, he said.

"NECC's transparency in dealing with the board since inception in 1998 demonstrates its good-faith intention to operate in compliance with the requirements of its license," Cirel said.

Besides possible state license violations, Biondolillo said the inspections also revealed "several health and safety deficiencies" at the NECC facility in Framingham, just west of Boston.

Three lots of steroids produced by the company are suspected in the outbreak, and the company shipped orders from those lots 13 times before receiving the results of its own tests to confirm those lots were sterile, Biondolillo said. Some medication was shipped as many as 11 days before the company received test results, she said.

Biondolillo also detailed signs of flawed sterilization procedures, including black specks of fungus in sealed vials of the steroids, which were returned to the company during a recall.

Investigators found the company didn't sterilize its products long enough and didn't adequately test whether its sterilization equipment was working, she said.

In addition, mats on which people wiped their shoes to remove contamination before entering a sterile environment were "visibly dirty and soiled with assorted debris," she said. And a leaking boiler adjacent to a pharmacy clean room left an unsanitary pool of water around it and the adjacent walls, she said.

None of what's been found is enough to definitively determine what caused the contamination, and the investigation is ongoing, Biondolillo said.

Meanwhile, Patrick's moves to increase oversight at the state's 25 compounding pharmacies have already started.

The first of the unannounced inspections, to take place at least annually, was done on Tuesday, health department spokesman Alec Loftus said. He wouldn't give the inspected facility's name and said the results are being reviewed.

Patrick said compounding pharmacies will now be required to file annual compliance reports that could help regulators determine if they are acting as manufacturers.

Philadelphia, October 23 Pediatricians and researchers at The Children's Hospital of Philadelphia's(CHOP) have developed a first-of-its kind tool to help parents and health care providers better treat ADHD (attention deficit-hyperactivity disorder). The new, three-part survey helps steer families and doctors toward "shared decision-making", an approach proven to improve healthcare results in adults, but not widely used in pediatric settings. The results of the CHOP study are published in the journal Academic Pediatrics.

"Shared decision-making in health care means that doctors and families make decisions together. Doctors contribute their professional knowledge, and families weigh their values and personal experience," explained lead author Alexander Fiks, M.D., M.S.C.E, an urban primary care pediatrician at CHOP and a faculty member at CHOP's PolicyLab. "We chose to focus on ADHD for this study, because it is a relatively common diagnosis with two recommended treatment options prescription medication and behavioral therapy that require the family to make decisions about what will work best for them. Choosing a treatment that doesn't 'fit' can lead to unsuccessful results. We wanted to see if we could create a tool to help guide families and physicians through this process."

According to a study published earlier this year, the number of physician outpatient visits in which ADHD was diagnosed in children under age 18 was 10.4 million. Psychostimulants were used in 87 percent of treatments prescribed during those visits.

The CHOP study involved 237 parents of children aged 6-12 who were diagnosed with ADHD within the past 18 months. Using a combination of parent interviews, current research, and input from parent advocates and professional experts, researchers developed a standardized three-part questionnaire to help parents define and prioritize their goals for treatment; attitudes toward medication; and comfort with behavioral therapies. The completed survey serves as a guide to support families and health care providers to reach the most effective and workable treatment for a child's ADHD.

"It's important to know whether a parent's primary goal is to keep a child from getting in trouble at school, improve academic performance, or maintain more peace with family members or peers," said Fiks. "We also need to learn about the family's lifestyle and attitudes toward behavioral therapy and medication. All of these factor into making the best treatment decision for each individual child and family."

Pediatric care providers do not usually use shared decision making to gauge families' preferences and treatment goals for ADHD. The Institute of Medicine and the American Academy of Pediatrics recommend healthcare providers use shared decision-making to help families select the best treatment option for ADHD. A well-designed questionnaire may help both patient-families and providers to feel more satisfied with their child's ADHD treatment. The researchers say this is a promising model for more widespread use to aid with treatment decisions for children with ADHD and, in the future, could be tailored for use with other medical conditions.

Until the approach is more widely adopted, Fiks and his colleagues recommend parents ask themselves a few questions to help get the most out of their office visit:

  • What do you and your child want to achieve as a result of ADHD treatment? (Better behavior or better grades at school? More self-control at home? Less teasing by other kids?)
  • Consider your attitudes and your family's attitudes about medication and behavioral therapy, and why they may or may not be right for you. Write down any questions you have about these treatment options to help remind you during your appointment.

"For the pediatrician's part, it is our responsibility to fully inform parents about their options for treating ADHD and to seek guidance from families about which options will best meet their treatment goals and be manageable for their lifestyle. Research shows that patients adhere much better to the treatment options that they are comfortable with and that are the most practical for them. We need to make sure we're asking the right questions," said Fiks, who is also co-medical director of CHOP's Pediatric Research Consortium.

Al-Qaida in Afghanistan is attempting a comeback

KABUL, Afghanistan (AP) ? A diminished but resilient al-Qaida, whose 9/11 attacks drew America into its longest war, is attempting a comeback in the country's mountainous east even as U.S. and allied forces wind down their combat mission and concede a small but steady toehold to the terrorist group.

That concerns U.S. commanders, who have intensified strikes against al-Qaida cells in recent months. It also undercuts an Obama administration narrative portraying al-Qaida as battered to the point of being a nonissue in Afghanistan as Western troops start leaving.

When he visited Afghanistan in May to mark the one-year anniversary of the U.S. raid that killed Osama bin Laden, President Barack Obama said his administration had turned the tide of war. "The goal that I set ? to defeat al-Qaida, and deny it a chance to rebuild ? is within reach," he said.

As things stand, however, an unquestionably weakened al-Qaida appears to have preserved at least limited means of regenerating inside Afghanistan as U.S. influence in the country wanes. The last U.S. combat troops are scheduled to be gone by Dec. 31, 2014, and security matters turned over to the Afghan government.

"They are trying to increase their numbers and take advantage of the Americans leaving," the police chief of Paktika province, Gen. Dawlat Khan Zadran, said through a translator in an interview this month in the governor's compound. He mentioned no numbers, but said al-Qaida has moved more weapons across the border from Pakistan.

For years the main target of U.S.-led forces has been the Taliban, rulers of Afghanistan and protectors of al-Qaida before the U.S. invasion 11 years ago. But the strategic goal is to prevent al-Qaida from again finding haven in Afghanistan from which to launch attacks on the U.S.

Al-Qaida's leadership fled in late 2001 to neighboring Pakistan, where it remains.

The group remains active inside Afghanistan, fighting U.S. troops, spreading extremist messages, raising money, recruiting young Afghans and providing military expertise to the Taliban and other radical groups.

U.S. Gen. John Allen, the top commander of international forces in Afghanistan, has said al-Qaida has re-emerged, and although its numbers are small, he says the group doesn't need a large presence to be influential.

U.S. officials say they are committed, even after the combat mission ends in 2014, to doing whatever it takes to prevent a major resurgence. The Americans intend, for example, to have special operation forces at the ready to keep a long-term lid on al-Qaida inside Afghanistan.

A more immediate worry is the threat posed by the growing presence of al-Qaida and affiliated groups in Yemen, Somalia and across a broad swath of North Africa, where it is believed al-Qaida-linked militants may have been responsible for the attack on the U.S. Consulate in Benghazi, Libya that killed U.S. Ambassador Chris Stevens and three other Americans.

U.S. and Afghan officials say al-Qaida also has been building ties with like-minded Islamic militant groups present in Afghanistan, including Pakistan-based Lashkar-e-Taiba, which is blamed for the November 2008 rampage in Mumbai, India, that killed 166 people, and the Islamic Movement of Uzbekistan, which is present in the north.

Ahmadullah Mowahed, a member of the Afghan parliament from the eastern province of Nuristan, along the Pakistan border, said he fears the departure of American combat forces will open the way for the Taliban and al-Qaida to overwhelm the provincial government.

"As soon as they leave, the eyes of al-Qaida will quickly focus on Nuristan," he said.

U.S. analysts say there is reason for concern that al-Qaida is down, but not out.

"They've been hit hard in a few cases, but they definitely are involved in the fight ? absolutely," said Seth G. Jones, a senior political scientist at RAND Corp.

Jones, a former adviser to the commander of U.S. special operations forces in Afghanistan, recently returned from a trip to eastern Afghanistan where he learned that al-Qaida's support network has expanded and its relations with groups such as the Pakistani-based Haqqani network are strong.

"That's a very serious concern because that kind of environment would allow al-Qaida to continue to operate, at least at a small level, because it's a workable environment for them, he said.

Richard Barrett, head of a U.N. group that monitors the threat posed by al-Qaida and the Taliban, said al-Qaida fears the Taliban will strike a deal with the Afghan government that would make the group all but irrelevant.

"So they will be doing whatever they can to assert their influence, to assert their presence" in Afghanistan, he said.

At least for now, al-Qaida in Afghanistan has no capability to launch attacks on the U.S., although commanders are taking no chances.

Little-noticed fragmentary U.S. military accounts of raids and strikes against al-Qaida in the northeastern provinces of Kunar and Nuristan show the group retains a command structure inside Afghanistan. On May 27 the U.S. killed the No. 2 commander in Afghanistan, Saudi national Sakhr al-Taifi, in Kunar, but it has yet to catch up to the top al-Qaida commander in the country, identified by U.S. officials as Farouk al-Qahtani, who resides in Nuristan.

In early September the international military coalition announced the death of an al-Qaida operative, Abu Saif, described as an associate of an al-Qaida leader killed along with several of his fighters July 1 in Kunar. Saif was called a conduit between senior al-Qaida leaders in Pakistan and Afghanistan, passing messages between them. In the most recent strike, officials said Sept. 27 they killed al-Qaida "facilitator" Abdul Rauf in Kunar. He was a Pakistani coordinator of foreign fighters' movements into Afghanistan and a builder of improvised explosive devices for attacks on coalition troops.

Interviews with Afghan officials suggest that al-Qaida also is present in other parts of the country, including the northwestern province of Faryab as well as Logar province, just south of Kabul.

Logar's provincial chief of police, Gen. Ghulam Sakhi Roogh Lawanay, said it is difficult to know how many militants are directly affiliated with al-Qaida, but he estimated their numbers in Logar at 100 to 150.

"Al-Qaida is very active. It is like fish. When one fish dies, another comes," he said in a recent interview. "The determination of these Arab fighters is high."

In interviews in Kabul and Washington, U.S. officials said they are satisfied that al-Qaida is so small inside Afghanistan ? they put the number at between 50 and 100 fighters ? that they can be contained indefinitely if the Afghan government allows U.S. counterterrorism forces to monitor and hunt the remnants. U.S. and Afghan officials are working to craft talks on a bilateral security agreement that could include such an arrangement.

Al-Qaida's numbers, however, don't tell the whole story.

Allen has said al-Qaida has learned to leverage its presence in Afghanistan to give the impression of having withstood U.S. military might and to burnish its image as a global force.

U.S. commanders say they will keep up pressure on al-Qaida to frustrate its goals, but few believe al-Qaida will be gone before U.S. troops leave.

"I see no evidence to suggest that it will be eliminated by 2014," said Jones, the RAND analyst.
